Permanent residence permit

  • Getting acquainted with the instruction and information about the procedure available in the MOS information section

  • Verification of whether there are grounds for refusing to initiate proceedings

  • Checking whether there are grounds for refusing to grant a permit, including whether the stay is considered uninterrupted within the meaning of the Act on Foreigners (* regarding selected grounds for granting the permit)

  • Filling in the application – in accordance with the rules and attaching 4 photographs that meet the criteria from the regulation

  • Payment of stamp duty for granting a permit

  • Personal submission of an application during legal residence (by appointment)

  • Presentation of the travel document at the time of application (and transmission of a photocopy of it)

  • Fingerprinting when applying

  • Attaching to the application the documents necessary to confirm the data contained in the application and the circumstances justifying the application for a permanent residence permit, therefore, in the case of:

Spouse of a citizen of the Republic of Poland:

current copy of the marriage certificate

documents confirming uninterrupted stay on the territory of the Republic of Poland

A minor child of a citizen of the Republic of Poland:

a copy of the child's birth certificate

photocopy of the ID card of a parent who is a citizen of the Republic of Poland

proof of the exercise of effective parental responsibility over the child

A minor child of a foreigner:

a copy of the child's birth certificate

photocopy of the parent's decision to grant a permanent residence permit / residence permit for a long-term EU resident

proof of the exercise of effective parental responsibility over the child

A foreigner of Polish origin

documents confirming the possession by at least one of the parents or grandparents or two great-grandparents of a foreigner of Polish nationality – according to the explanation provided in the MOS

documents confirming the connection with Polishness, e.g. certificates of membership in Polish diaspora organizations.

documents confirming the intention to settle in Poland, including a statement of intention to settle

Holder of a valid Pole's Card:

a copy of the Pole's Card (original for inspection at the application stage)

documents confirming the intention to settle in Poland, including a statement of intention to settle

Victims of trafficking in human beings:

evidence of uninterrupted stay in Poland immediately before submitting the application or justifying breaks in stay

documents confirming cooperation with law enforcement authorities in criminal proceedings in the case of the crime of trafficking in human beings;

confirmation of the foreigner's fears of returning to the country of origin

A foreigner who has been granted asylum in Poland:

Copy of the UdSC Head's decision to grant asylum (original for inspection)

A foreigner who has been granted refugee status in Poland has been granted subsidiary protection, permission to stay for humanitarian reasons or tolerated stay

Documents confirming the foreigner's possession of refugee status, subsidiary protection, consent to stay for humanitarian reasons or consent to tolerated stay

Evidence of uninterrupted stay in Poland or justifying breaks in stay

  • Attaching a power of attorney with proof of payment of stamp duty (if a representative has been appointed)
